Recording an accident report
Record an accident, the first aid you gave and who you told, then download the accident report as a PDF to share with parents.
Who this is for
Who this is forChildminders and assistants recording an accident that happened in their setting. Needs a Solo or Team plan, or a free trial. It takes about five minutes.
When a child is hurt in your care, you need a written record of what happened, what you did and who you told. KinderStart turns the form into a signed accident report you can download and share.
1. Start a new accident form
Open the Tools tab, tap Health & Safety Logs, then Accident Reports, and tap +.
2. Say what happened
Choose the child, then check the Date and Time. Set the Severity Level to Minor or Serious.
Fill in the Location, then write What happened?, for example "Tripped over the rug edge and landed on her knee". Tap the kind of injury (Graze, Bruise, Cut, Bump to head, Burn, Bite or Other) and add Injury Details if there's a mark.
3. Record the first aid
Check Who gave first aid? (it starts with your name), answer Medical Advice Sought? and describe the Action Taken, such as cleaning a graze and applying a cold compress. Add any witnesses under Witness Info.
4. Save it
Tap Save Accident Form. The report appears at the top of Accident Reports, marked Pending Signature until the parent signs it in their portal.
5. Download the report
Tap the download button on the report's card. KinderStart builds the accident report as a PDF with your logo, the details you recorded, the body map if you added one, and the signatures so far. Print it, save it or share it with the child's parents.
Made a mistake?
Open the report and tap the pencil to edit it, or the bin to delete it. Deleted reports can be restored from
Recently Deleted.
