About this form

This printable is for childminders in England who need to complete the EYFS progress check for a child between the ages of 2 and 3. It gives you one clear place to write a short summary of how the child is getting on, in your own words.

The form follows the three prime areas: communication and language, physical development, and personal, social and emotional development. For each one you note the child's strengths and progress, how you will support them, and how the family can help at home. A second page records the parent or carer's views, the plan you agree together, and who will help next if there is a concern.

It's meant to be filled in with the family, perhaps at pick-up or at a short arranged chat. Once it's signed, you can share it with the health visitor so it sits alongside the child's health and development review.

What's inside

  • Child's details: name, date of birth, age in months, date of check, setting and childminder
  • Communication and Language: strengths and progress, how we will support them, how you can support at home
  • Physical Development: strengths and progress, how we will support them, how you can support at home
  • Personal, Social and Emotional: strengths and progress, how we will support them, how you can support at home
  • Parent / carer voice: what the family sees at home and wants you to know
  • Our agreed action plan: next steps, who does what, when you will look again, and who will help if there is a concern
  • For the health visitor: dates shared with the family and sent to the health visitor, and the parent's agreement to share
  • Signatures and dates for the childminder and the parent or carer

How to use it

  1. Print one copy for each child who is due their check between the ages of 2 and 3.
  2. Before you meet the family, jot down what you have seen of the child's strengths and progress in each of the three prime areas.
  3. Sit down with the parent or carer and add their view of the child at home in the parent / carer voice box.
  4. Agree the next steps together: what you will do, what the family will do, and when you will look again. If there is a concern, note who will help next, such as the health visitor.
  5. Both sign and date the form, then record when you shared it and, with the family's agreement, when you sent it to the health visitor.

Can I use it for several children?+
